Contact Form Reminder:
Thank you for turning in your
updated Annual Health/Emergency Contact Information Form if you have already
done so this school year.
If you have not yet done so, please do one of the following:
· fill
out the attached form (front and back side) and return it to the health
office as soon
as possible OR fill out the hard copy of the form which will be sent home by Friday
of this week with your student.
