Annual Health/Emergency Contact Letter
For the 2015-16 school year the Annual Health/Emergency form will
be sent to your home before the end of this school year. Mail or return the form by the first day of
school. Please send it to “attention: health
office” of the school where your student will be attending next fall.
Emergency
Action Plans
If your student has asthma, seizures, diabetes, severe food
allergies or bee sting reactions, we request completion of forms specific to
these conditions annually. They will be
sent home with your student before the end of this school year. Please return them to school next August with
medications and doctor orders, as needed.
End of Year Medication Pick-up
If your child has emergency
medication at school this year (i.e. Epi-pens, inhalers, etc.), we
recommend leaving the medication at school until dismissal time on the last day
of school (June 11th). If
your child has non-emergency medication
at school, you may pick it up any time before school dismissal on June 11th. Medications that are not picked up by June 11th
(dismissal time) will be discarded.
Controlled substances cannot be sent home with
students. Call
your schools Health Office if you wish to make other arrangements.
Medication for Next
School Year
Should your child require medication next fall (including
Tylenol, Ibuprofen, dietary supplements, non-prescription medications, lotions,
etc.), please have your Physician (or licensed prescriber) fill out the front
portion of the medication authorization form (found on the district website) at
your convenience this summer. A parent
or guardian must sign the back of the medication authorization form. If your student had medication and/or
emergency action plans in the health office this year, new forms for next year
will be sent home before the end of this school year.
