Any
medications brought to the health office this year for your student will need
to be picked up by 3:00 p.m. on June 11th.
Non-emergency medications (i.e. Tylenol, etc.) may be picked up earlier
than June 11th. Controlled medications
cannot be sent home with middle school students.
Should your
child require medication for the next school year (including Tylenol,
Ibuprofen, Motrin, dietary supplements, non-prescription medications, lotions,
etc.), please have your physician (or
licensed prescriber) fill out the front portion of the medication
authorization form (found on the
district website) at your convenience this summer. A parent or guardian must sign the
back of the medication authorization form.
If your
student had medication and/or emergency action plans in the health office this
year, new forms for next year will be sent home with your student before the end of this school year.
